Submit a CΨPRS Assessment
This page is for field supervisors from our current partner institutions, and institutions that have an annual subscription for the online CΨPRS to submit mid- and end-placement assessments.
Online CΨPRS evaluations can only be completed for students enrolled in the following institutions. Click on the mid- or end-of-placement assessment links as appropriate to complete your assessment.

If you are a supervisor completing an assessment for a bona fide trainee of one of the above mentioned institutions and for some reason, you prefer to complete a hard copy version of the CΨPRS (in lieu of the highly recommended online version), contact the Clinic Director/Practicum Coordinator of the concerned institution.
Participation in our Research
Research on the CΨPRS is an ongoing endeavour, and much progress has been made. If you are happy to contribute to the ongoing research, please review the information sheet available at the beginning of the online assessment and then accept the research invitation. If you prefer that your ratings be excluded from the research, please decline the research invitation.
